Family medicine doctors’ attitudes regarding working in public and private facilities

Abstract:
Entrepreneurial initiative among family medicine doctors in the Republic of Srpska is underrepresented, as private owned special clinics for family medicine provide services to less than 5% of health-insured people. The study aimed to compare attitudes of family medicine doctors (FMDs) working in public and private facilities and to compare reasons represented by FMDs working in private facilities, why FMDs work in public facilities instead of in private facilities. The study used exploratory sequential mixed research methods, i.e. two-stage models, with quality and quantitate part of the research. A qualitative part of the research was conducted in a privately owned health facility on a sample of 15 respondents using an interview guide. The quantitative part of the research was conducted on a sample of 121 FMDs employed in public health centers, using a questionnaire. The results showed that more than 1/3 of FMDs working in public facilities thought that is better to work in public instead of a private facility, while 50% of FMDs were not sure where is better to work. Further, the results showed that more than 50% of FMDs thought that: “management is not their responsibility”, “pay is guaranteed no matter on the number registered population and contract with insurance fund”, and “in the public facility could organize better their own time”. The FMDs attitudes in private facilities were different from the FMDs attitudes work in public facilities. But the reasons listed by FMDs working in private facilities, why their colleagues work in public rather than in private facilities were almost identical. Instead of a conclusion, these results are very important to policymakers and managers to create an entrepreneurial culture in public health centers and create an entrepreneurial climate in the health care system.
